Interview with Dr. Tom Mammone, ED

Question: What are pores and what is their function?

Dr. Tom Mammone: Pores have an important function in skin physiology - they provide the openings for three different glands: the apocrine, sweat, and sebaceous glands. Apocrine glands secrete hormones which are most active during the onset of puberty; sweat glands help the body to cool itself, responding to elevated body temperature; and, sebaceous glands, which are the most noticeable, secrete sebum and are associated with the hair follicle. The hair follicle within the pore allows the oil generated in the sebaceous glands to reach the surface and moisten the skin. Pores tend to be more visible on the parts of the face where oil the oil glands are bigger - the nose and forehead.


Question: Are there genetic factors that influence pore size?

Dr. Tom Mammone: The size of your pores can depend on many factors, including your skin tone and type. Lighter skin tones and drier skin types tend to have smaller pores whereas olive and dark skin tones and oiler skin types tend to have more noticeable pores. Pores can also appear larger in darker skin types due to the accumulation of melanin in the debris that clogs the pore. There is also a direct correlation between pore size and sebum production - too much sebum creates enlarged pores.


Question: How do pores become enlarged?

Dr. Tom Mammone: As we age, the cell renewal and maturation process becomes irregular which can result in a compromised skin barrier and rough, uneven skin texture. Sun damage causes inflammation that can promote excessive oil production as well as weaken the collagen and elastin within the deeper layers of the skin that support pores. Picking at your skin and squeezing a pimple can also cause inflammation and permanently damage to the pore walls. Pore walls can be stretched over time due to clogging from excess oil production and cellular debris, such as dead skin cells that build up around the edges of the pores, making them appear larger than they really are. Finally, dehydration causes skin cells to flatten, increasing the gaps between cells and adding to the appearance of enlarged pores.


Question: Do pores age like the rest of the skin?

Dr. Tom Mammone: Recent studies show that pores appear to age like the rest of the skin by growing both larger and longer, forming a more elliptical shape, as we get older. This is due to both excessive sebum production as well as sagging skin that results from long-term damage to the underlying skin structure.


Question: Are there any preventative steps to take?

Dr. Tom Mammone: The easiest, most basic steps we can take to help reduce pore size are to cleanse the skin daily to remove excess oil and cellular debris that accumulates in the pore then follow with gentle exfoliation to remove any additional dead skin cells. Finally, apply a light moisturizer to plump and hydrate the skin. Ensuring that your skin's moisture barrier is hydrated with an intact skin barrier is essential to maintaining the skin structures that support the walls of the pore. If this skin is dehydrated then the pore will appear enlarged. This process of cleansing, exfoliating and moisturising will help the pore to "snap" back to its original size.
